Hello, I've been working on a mod called the Montgomery County Special Operations Mod based off of Montgomery County, MD. This mod is a submod of SlateDragon1's West Falls Modification, but the map and units have all been edited, with many new units added. My team, jakecwri and thepianistwebbolide and me, have worked for the past couple of months. We hope to have a beta released sometime soon. Here are some screenshots from the mod.

What is the process of making new equipment for a em4 mod? I would like emts to be able to grab more equipment from a ambulance.

Like how your able to grab a med bag from the ambulance

You would need to edit the unit.xml to allow for the person to be available for use.

P.S i heard if a firefighter paramedic heals without a medic bag it can fully heal a injured person and you don't need to heal them again. and would be unaffected by any damages including fire and chemicals.

Just add the heal command to the unit and edit its traits. No need to do anything crazy

First you make a screenshot of the units in the editor. If you want to crop the silhouettes, I'd recommend changing the floor texture to something like the one below. Once you've cropped everything, you can use this tool; it's not perfect, but it simplifies everything.
